We have been starting our neuroimmune axis patients on low doses and then working up if needed. Many have noticed improvements. Good luck with the trial!

3. I'm actually an expert on topic X. LLMs produce random results that are less than useless (actually dangerous in many cases). Models designed for specific use cases can be very powerful. However, general AGI doesn't exist.

Usually a trial with Vyvgart results in significant disease modification if it is caused by IgG. In some cases the disease resolves, in others anti-CD19 therapies can get remove the plasma cells producing the pathogenic autoantibody.

Most people with autoantibody driven disease are seronegative. Autoantibody tests really suck. Scientists are working on better ones, however it's a nontrivial task to make inexpensive high throughput assays that can accurately detect a wide array of pathogenic autoantibodies for any protein.

After seeing a lot of patients in the neuroimmune axis disorders space (which includes many post-infectious illnesses) it's pretty easy to identify people who likely have autoantibody driven disease.

Off label just means that it is prescribed for a condition other than what it was FDA approved for. Vyvgart has been approved for a few autoimmune conditions, however it can be used for many more. Currently, if it's not covered by insurance it can be very expensive off label (~$100k per year).

The trial was poorly designed. However, Vyvgart worked really well for those with autoimmune phenotypes. We use it for a wide range of conditions off label (there are tons of conditions caused by IgG autoantibodies that are seronegative). The results are really impressive.
