Fallout: New Vegas's DLC uses scripts to inject its items into relevant FormLists so that its weapons are affected by perks and so on. These scripts are also responsible for displaying the "DLC Loaded" messages when you leave Doc Mitchell's house after the intro. 1/2

In The Labyrinth of Time, entering the Ziggurat without a lantern is supposed to show a black screen. However, the developers put in the wrong filename for the black screen, so it fails to render, showing a black screen instead
