Clinically stroke, even a TÍA (transient ischemic attack) is less likely since we've not seen a marked change or specific event to suggest that. Much more likely is a cognitive neuro-degenerative process like Alzheimer or frontotemporal dementia.

He appears to be losing more of the self-control that frontal lobes provide (releasing twice the AI video of the plane dropping....on the No Kings crowd).
If either is the case, his people will never release the MRI results until he's caught drooling while sitting with people.

Whoever suggested hand bruises from an infusion that leaked (extravasated) may be on the right track, that he's receiving one of the Alzheimer drugs to slow down the progress of the deterioration....even though most of the drugs are rather ineffective.
