Perspectives on technology: All STEPS count – an integrated framework for net zero urological care Objective To present a narrative review of evidence to guide the delivery of high-quality, low-carbon urological care using a structured framework. Methods Academic and policy papers which outline actions focused on decarbonising urological care and surgical care more broadly were identified and reviewed. The ‘STEPS to Low-Carbon Care’ framework (an acronym for low-carbon care across ‘Settings and Treatments, Efficiency, Prevention and System change’) was then used to categorise and present the evidence-based decarbonisation actions, using the National Health Service in England as a case study. Results Across all STEPS framework elements, tangible actions were identified alongside opportunities for future research and innovation. The evidence-based actions that were identified to transition to low-carbon care settings and treatments included tackling known carbon hotspots in operating theatres: anaesthetic gases, consumables and electricity use. Outside the operating theatre, urology pathway transformation through one-stop clinics, day-case surgery, appropriate use of virtual appointments and streamlined pathways demonstrated opportunities to reduce carbon emissions, with potential additional benefits in terms of cost, efficiencies, and patient outcome improvements. Key climate mitigation actions that support keeping people healthy were identified. There was a paucity of evidence demonstrating the implementation of climate change action as part of routine service delivery. Embedding sustainability across organisational processes and ways of working requires actions to upskill, engage and enable the workforce to deliver and to establish clinical leadership. Conclusion This review identified a range of interventions to decarbonise urological care, whilst highlighting a need for further research. Abstract Objective To compare functional and surgical outcomes of robot-assisted ileal ureter replacement (RAIUR) vs robot-assisted kidney autotransplantation (RAKAT). Patients and Methods This was a retrospective analysis of patients who underwent RAIUR or RAKAT for long ureteric strictures at eight European tertiary centres (2017–2024). Primary endpoints were maintenance of estimated glomerular filtration rate (eGFR) and postoperative complications within 30 days of surgery, described using the Clavien–Dindo classification. Secondary outcomes included infections, need for lifelong drainage, stricture persistence, and re-intervention for the stricture during follow-up. Results A total of 15 and 39 patients underwent RAIUR and RAKAT, respectively. The patients who underwent RAIUR were older (61 vs 45 years, P = 0.03), with more comorbidities (Charlson Comorbidity Index ≥3: 67% vs 28%, P = 0.03) and had a lower baseline eGFR (60 vs 87 mL/min/1.73m2, P = 0.007). The median stricture length was 9.6 cm for RAIUR vs 7 cm for RAKAT. Patients who underwent RAIUR had a shorter surgical time (290 vs 355 min, P = 0.008), whereas those who underwent RAKAT had a shorter hospitalisation (5 vs 8 days, P = 0.001). Overall complications were higher after RAIUR (73% vs 31%, P = 0.01), but Clavien–Dindo Grade ≥III complications were similar (13% vs 10%). During follow-up, both groups showed slightly improved renal function, low infection rates (13% for RAIUR vs 10% for RAKAT), low stricture persistence (13% for RAIUR vs 7.7% for RAKAT), and minimal need for drainage or re-intervention. The main limitation was the modest sample size.

Abstract Objectives To evaluate the results of randomised controlled trials (RCTs) regarding the efficacy of human papillomavirus (HPV) vaccination in preventing male genital-related diseases. Methods A systematic search of English language literature using PubMed, Scopus, and Cochrane Library was performed in April 2024 according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) protocol. Results Evidence from four RCTs (including 7008 male participants) support the efficacy of the quadrivalent HPV vaccine in preventing genital warts and persistent HPV infection in HPV-naïve men. The low incidence of male genital malignancies in the control groups of the reported studies lead to underpowered evidence. However, vaccination leads to durable protection with a long-term follow-up of 10 years showing efficacy of 91.8% to prevent HPV 6-, 11-, 16-, or 18-related external genital lesions (EGLs) in HPV-naïve subjects. Additionally, the quadrivalent vaccine seems to effectively reduce the detection of DNA from all four HPV types. Conclusion In summary, early quadrivalent HPV vaccination demonstrates efficacy in preventing HPV infection and EGLs in males. Well controlled prospective studies are needed to confirm the long-term efficacy, specifically in cancer prevention, in all men and specific subject subgroups, and to identify the targeted population who is most likely to benefit from early vaccination.

Abstract Objectives To evaluate the long-term functional outcomes of robot-assisted partial nephrectomy (RAPN) in patients with and without a solitary kidney. Materials and Methods A retrospective review was performed using the prospectively maintained, institutional review board-approved database to identify all consecutive patients who underwent RAPN between 2006 and 2023. A propensity-score-matched analysis was subsequently undertaken to compare patients with and without a solitary kidney. The analysis controlled for various baseline clinicodemographic variables, including age, comorbidities, and baseline tumour characteristics. Results Of the 1698 RAPN patients included in our series, 86 were identified to have a pre-existing solitary kidney. Following a 1:1 propensity-score-matched comparison analysis, the solitary kidney cohort was noted to have similarly favourable long-term functional outcomes to the cohort with bilaterally functioning kidneys. This was also indicated by our Kaplan–Meier analysis, which highlighted identical survival from kidney disease progression at a median follow-up duration of 45 months (P = 0.101). Improvements in functional outcomes could be observed as early as 3 months after the respective surgery (median glomerular filtration rate preservation in the solitary vs non-solitary kidney group: 63.7% vs 64.7%; P = 0.815). Of note, the solitary kidney group was more at risk of postoperative acute kidney injury (AKI; solitary vs non-solitary kidney group: 79.7% vs 57.7%; P < 0.001). Lower preoperative renal function (odds ratio [OR] 0.963, 95% confidence interval [CI] 0.922–0.991; P = 0.028), main artery clamping (OR 5.015, 95% CI 1.044–27.462; P = 0.027), and ischaemia time beyond 25 min (OR 4.335, 95% CI 1.381–13.846; P = 0.014) were identified as independent predictors of AKI.

Abstract Objectives To evaluate the impact of advocacy efforts by organisations such as the Bladder Cancer Advocacy Network (BCAN) to increase awareness about the signs, symptoms, diagnosis, treatment and need for a multidisciplinary approach to the care of patients with muscle-invasive bladder cancer (MIBC). Materials and Methods We developed a 32-question survey with input from physicians, nurses, patients and caregivers. The survey was posted on the BCAN website between August 2013 and September 2014 (Cohort A) and again between June 2023 and April 2024 (Cohort B). Questions focused on time from initial symptoms to diagnosis and treatment, proportion seeing a medical oncologist or radiation oncologist, treatments offered/received, and patient satisfaction with their treatment choices. Results Overall, 337 self-selected patients mostly white males from the US or Canada, with at least an undergraduate education completed the survey. There were 243 patients in Cohort A and 94 patients in Cohort B. The median age (range) at diagnosis was 61 (31–93) in Cohort A and 63 (38–87) in Cohort B. The most common presenting symptom was hematuria. In Cohorts A and B, 35% vs 30% (P = 0.56) waited >3 months to seek medical attention, and in 38% vs 43% (P = 0.76) it took >3 months to obtain a diagnosis. Men were more likely than women to be diagnosed within 1–2 months in both cohorts (Cohort A: 68% vs 47%; P = 0.03, Cohort B: 70% vs 37%; P = 0.03). Preoperative consultation with a radiation oncologist and bladder-sparing use were infrequent. More patients in Cohort A (77%) felt they had enough time to make decisions compared to Cohort B (67%) (P = 0.004). Most patients were satisfied with their treatment choices.

Abstract Objective To evaluate the performance of Bladder Epicheck® (BE; Nucleix Ltd., Rehovot, Israel) in predicting tumour recurrence and bacillus Calmette–Guérin (BCG) failure during the first year after induction treatment. Patients and methods Prospective study including 65 patients with non-muscle-invasive bladder cancer treated with BCG between 2018 and 2021. Urine samples analysed with BE were collected before and after BCG induction. Logistic binary regression was used to assess the association between clinical and pathological variables and BE results with tumour recurrence and BCG failure during the first year after induction treatment. Results During follow-up, 16 (24.6%) patients experienced a bladder cancer event, 11 (68.8%) of which were BCG failure (high-grade recurrence) and five (31.2%) were low-grade recurrences. The median (range) time to overall recurrence was 7.3 (3.8–17.4) months. A significant association was found between the risk of tumour recurrence/BCG failure and post-BCG cystoscopy (odds ratio [OR] 10.0; P < 0.001 and OR 13.1; P < 0.001, respectively), post-BCG BE result (OR 16.9; P < 0.001 and OR 33.1; P < 0.001, respectively) and pre/post-BCG EpiScore value variation (OR 14.4; P = 0.001 and OR 7.1; P = 0.018, respectively). A nomogram including these three variables outperformed the Club Urológico Español de Tratamiento Oncológico (CUETO) risk tables to predict any bladder cancer event after BCG induction (area under the curve 95.1% vs 67.1%). Result validation in a larger and independent series is needed. Conclusions The BE post-BCG status and variations in EpiScore values can help us identify patients at higher risk of any bladder cancer event and BCG failure promptly. These data can have an impact on disease management.

Pelvic organ-sparing #RARC demonstrates comparable oncological and complication outcomes to non-#organsparing RARC in well-selected women with #BladderCancer and it could be considered more often with aim to improve functional outcomes without compromising oncological results.
