The state of care for people with multiple long-term conditions (MLTCs) is poor.

A holistic approach is often lacking, with poor communication and limited or non-existent coordination that often falls on the individual.

The 10 year health plan acknowledges this. What solutions does it offer?

Great morning - international comparisons, exceptionalism (or lack thereof) and dwindling political opportunity all well discussed
Thank you to all the excellent speakers and delegates who took part in our #hospitaltohome policy event this morning - we heard examples of successful care shifts in other countries, and wrapped up with calls for greater visibility and parity for care closer to home.

Talking social care reform:

- Govts rarely get more powerful over time
- Social care reform may burn political capital (costs a lot and ~50% of public think it’s funded already)
- But it needs to happen & we know what to do

So…waiting=frustrating
