No clear differentiation between hypoxic TCA (TBI) -> CCs useful following airway vs hypovol/obstructive (tamponade/tensionPtx) TCA -> CCs usually not useful but no good evidence behind it.
If prehosp teams cannot perform TCA algorithm -> rapid transport to right hosp.

#ERC2025 are out with some interesting specifications esp @trauma
- enforcement of clinical priorities and treatment of rev causes OVER CCs (+de-enforcement of #REBOA)
- causes and pathophysio of #TCA (also met acidosis)
- role of vasopressors
- CCs specified (usually not useful)

Achievement of hemostasis is associated with significant coagulation, transfusion volumes and mortality benefits. Early whole blood resuscitation is associated with a greater independent odds of achieving hemostasis and at an earlier time point over the first 4 hours.

There is increasingly evidence that diastolic blood pressure (DBP) is a crucial resuscitation target in trauma and cardiac care. Based on a great article by Dan and Rich we look at how optimizing DBP may improve survival rates in emergency medicine.

This week I read a great article in the EMJ by Dan Horner and Rich Carden on new resuscitation targets. notably the use of diastolic blood pressure. DBP is the prime driver of coronary perfusion and thus successful resuscitation. Not only in cardiac arrest, but seemingly also in trauma. This blog (with their permission and review) outlines the main points from that article and links their thoughts on trauma to similar emerging evidence in cardiac resuscitation.
